UPMC Community Medicine Incorporated - Full-Time Professional Staff Nurse, OP

UPMC Community Medicine Incorporated is hiring a Full-Time Professional Staff Nurse, OP to support Pittsburgh Primary Care Brentwood.

Hours: Monday-Friday, rotating daylight shifts

Location: Brentwood with occasional travel to Greentree and Pleasant Hills

Department Details: This large primary care practice has 3 locations, serving patients from age 18 through lifespan. It includes 6 physicians and 5 advanced practice providers. No holidays or weekends are required. Responsibilities include quality improvement activities, post-hospital discharge follow-up, and medication reconciliation, primarily using EpicCare software, telephone triage, medication refills, coumadin management, result discussion, and patient education.

The Professional Staff Nurse is a Registered Nurse responsible for setting standards of care, managing, and providing patient care activities for a group of patients and their families through independent judgment, communication, and collaboration. The role involves establishing collaborative relationships with healthcare team members to achieve optimal patient outcomes and demonstrates a commitment to the community and nursing profession.

Responsibilities:
