Qualifications

• Bachelor’s degree in Nursing or Social Work (or current licensure) required.
• Strong case management and care coordination skills.
• Experience in elder care, social services, human services, nursing, or related field.
• Excellent communication, organization, and time-management abilities.
• Ability to manage multiple cases and priorities effectively.
• Comfort working in a professional office setting with basic computer skills.
• Certification in social work, nursing, gerontology, or related field is a plus.
