Job summary

CenterWell Senior Primary Care, part of Humana, is hiring a Physician Collaborator for a fully remote role supporting fracture care in seniors. You will collaborate with APPs to close gaps in care, review charts, and coordinate monthly meetings, reporting to the Associate Director.

The position offers a highly flexible, mostly remote schedule with occasional travel. Requirements include an unrestricted medical license and residency in South Carolina, MD/DO from an accredited university, board

Qualifications

As a Physician Collaborator you will share our vision to improve health care in the communities we serve! This is a fully remote role, with an average of 5 hours of work monthly. You will report to our Associate Director.
You will collaborate with our APPs who provide virtual wraparound services to support patients who have had fractures in the last 6 months when they cannot get in with their own PCP.

Required Qualifications

• Must have unrestricted medical license and reside in the state of South Carolina
• Graduate of MD/DO program of accredited university
• Board certification or Board Eligible
• 2+ years of experience collaborating with mid-levels

Work at Home Requirements: To ensure Home or Hybrid Home/Office employees’ ability to work effectively, the self-provided internet service of Home or Hybrid Home/Office employees must meet the following criteria: At minimum, a download speed of 25 Mbps and an upload speed of 10 Mbps is required; wireless, wired cable or DSL connection is suggested. In certain roles, the minimum recommended internet speed required by Humana may not be sufficient for business needs. Humana reserves the right to require associates to upgrade their internet service if necessary. Work from a dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information.
Travel: While this is a remote position, occasional travel to Humana's offices for training or meetings may be required.
