Memorial Health is seeking an Urgent Care Advanced Practice Provider (NP or PA) in Decatur on a PRN basis. Compensation ranges from $65.91 to $76.80 per hour, with responsibilities spanning pediatric to geriatric care under physician supervision and collaboration for patient advocacy and education.

Licensure requirements include IL APRN/PA licenses, DEA and controlled substances where needed, CPR, and ACLS if required. Minimum 1 year post-graduate experience preferred.

Qualifications

• APRn license required by Illinois State Law.
• PA license required by Illinois State Law.
• Appropriate APRN/PA certification as determined by department.
• IL Controlled Substance license must be obtained within 3 months of hire if not held.
• DEA license must be obtained within 3 months of hire if required.
• CPR certification required; ACLS where required.

Responsabilités

• Assess, diagnose, and treat patients in collaboration with supervising physicians.
• Follow protocols and coordinate patient follow-ups with primary care providers.
• Document timely and accurately per Medicare/Medicaid and MHS policies.
• Coordinate resources required by patients and support care plans.
• Collaborate with other health care professionals to formulate and initiate care plans.
• Develop and evaluate plans for adherence to quality indicators and outcomes.
