Job summary

emmUcare Home Health is seeking a PRN Home Health Aide to provide compassionate, in-home personal care across multiple patient homes in Pennsylvania. The role requires travel between locations and flexible scheduling to meet patient needs.

Responsibilities include assisting with ADLs, mobility, light housekeeping, meal support, and careful documentation. A valid HHA training background, CPR, and driving capabilities are preferred for this field role.

Qualifications

• High school diploma or GED preferred.
• Completed state-approved Home Health Aide training program (if required).
• CNA certification preferred but not always required.
• Current CPR certification preferred.

Responsibilities

• Assist patients with activities of daily living (ADLs) including bathing, grooming, dressing, and toileting.
• Provide mobility support, including transfers, ambulation, and positioning.
• Perform light housekeeping (laundry, dishes, tidying patient areas).
• Prepare simple meals and assist with feeding when needed.
• Monitor and report changes in patient condition to supervising nurse or agency staff.
• Maintain accurate documentation of care provided.
• Follow individualized care plans and safety protocols.
• Travel to multiple patient homes within assigned geographic area.

Job Title: PRN (Per Diem) Home Health Aide (HHA) - Field/Travel

Location: Pennsylvania (various patient homes within assigned service area)

Position Summary:

The PRN Home Health Aide provides compassionate, hands-on personal care to patients in their homes on an as-needed basis. This role requires travel between patient locations and flexibility in scheduling. The aide supports patients with daily living activities while promoting comfort, dignity, and independence.

Key Responsibilities

• Assist patients with activities of daily living (ADLs), including bathing, grooming, dressing, and toileting
• Provide mobility support, including transfers, ambulation, and positioning
• Perform light housekeeping (laundry, dishes, tidying patient areas)
• Prepare simple meals and assist with feeding when needed
• Monitor and report changes in patient condition to supervising nurse or agency staff

Schedule:

• PRN / Per Diem (as needed) required four shifts per month
• Flexible shifts based on patient demand
• May include days, evenings, weekends, and holidays

Qualifications:

• High school diploma or GED preferred
• Completion of a state-approved Home Health Aide training program (if required)
• CNA certification (preferred but not always required)
• Current CPR certification (preferred)
• Valid driver’s license, vehicle, auto insurance, and valid registration
• Ability to pass background check and drug screening

Compensation & Benefits

• Hourly pay (typically $16-$18/hour depending on experience and assignment)
