Job summary

Boundless Home Health LLC seeks a compassionate PRN Home Health Aide to provide in-home care on a flexible schedule. You will assist with personal care, support daily living activities, and follow care plans under RN supervision.

The role emphasizes documentation accuracy, patient safety, and a willingness to travel to patient homes. BLS certification is required; Home care experience is preferred. Pay is per visit with variable weekly hours.

Qualifications

• Good customer service skills required.
• BLS Certification required (per license/certification section).
• Home care experience preferred.

Responsibilities

• Assist patients with personal care activities (baths, grooming, oral hygiene) and follow care plans.
• Notify RN of changes in patient condition and document promptly.
• Record and maintain accurate patient charts per protocol.
• Demonstrate flexibility with duties and participate in additional tasks as needed.

We are seeking a compassionate and independent PRN Home Health Aide to join our dedicated healthcare team.

Responsibilities

• Perform and/or assist patient with personal care activities, such as, baths, showers, skin and hair care, and oral hygiene. Assure universal safety precautions and direction on the care plan are followed when providing care. Provide care that meets the patient’s needs.
• Notify RN of any changes in patient condition observed in a timely manner and document appropriately.
• Record all required information on patient chart completely and accurately. Assure documentation is completed per organization protocol.
• Other duties and/or projects as assigned. Demonstrate flexibility with job responsibilities in all areas.
