Qualifications

• Requires PGY1 residency or 3 years hospital/pharmacy experience.
• One year of experience precepting pharmacist students or residents.
• Graduate of an education program approved by the credentialing body for required credentials.

Responsibilities

• Verify and process orders, provide medication therapy review and patient care services.
• Counsel patients and provide drug information; perform discharge counseling as needed.
• Mentor technicians, students and residents; coordinate shift activities and education materials.
• Participate in quality, safety, and service improvement initiatives across the department.
