Qualifications

• Graduate of education program approved by the credentialing body for the required credential(s) indicated below.
• ASHP-accredited PGY1 residency or equivalent required, or three years hospital pharmacy experience.
• Experience in precepting students or residents preferred.

Responsibilities

• Provide day-to-day patient-centered pharmacy services across departments.
• Review patient profiles and perform discharge counseling and medication histories.
• Collaborate with healthcare teams to optimize therapy and safety.
• Precept pharmacy students and residents and mentor staff.
