(PRN) Speech Therapist (Limestone County, AL) - ProHealth Home Health & Hospice
A Speech Language Pathologist (SLP) administers speech therapy to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the supervision of the Director of Clinical Services/Nursing Supervisor. Services are provided only by or under the supervision of a qualified speech pathologist or audiologist.
Qualifications
- A master’s or doctoral degree in Speech Language Pathology, licensed as a Speech Language Pathologist in the state of practice.
- Or, successfully completed 350 supervised clinical practicum hours (or in the process), at least nine months of supervised full-time SLP experience, and passed a nationally approved exam.
- Currently licensed in the state(s) of practice.
- Preferred: Two (2) years’ experience.
Responsibilities
- Improve or maximize the patient’s communication abilities.
- Assist the patient in returning to optimal and productive living within their capabilities.
- Participate in patient care planning with home care personnel.
- Provide the full range of Speech Language Pathology services as ordered by the physician.
- Supervise personnel as assigned.
- Conduct initial history-taking and evaluation.
- Perform all skilled procedures.
- Consult with physicians regarding treatment changes.
- Report patient progress to physicians.
- Instruct patients and families in home programs.
- Present in-service training to staff periodically.
- Complete initial evaluation, treatment plan, and goals; submit for physician approval. Conduct recertification every 60 days if treatment continues. Write progress notes after each visit, reevaluations at expected treatment end, and discharge summaries.
- Make supervisory visits every fourteen days for patients seen by home health aides.
- Assist physicians in evaluating the patient’s functional level.
- Develop and revise the plan of care as needed.
- Prepare clinical and progress notes.
- Consult with family and agency personnel.
- Participate in in-service programs.
- Complete and submit OASIS assessments, transfers, resumption of care, discharges, and significant change reports within agency timeframes.
- Utilize ICD-10 codes appropriately.
- Participate in QAPI activities as assigned.
Working Environment
Work indoors in the agency office and patient homes, with travel between locations.

- Participate in physical activities as needed.
- Work for extended periods standing and involved in physical activity.
- Moderate lifting required.
- Regular bending, lifting, and standing.
