Job summary

Windsor Rehabilitation And Healthcare Center in Windsor, NC is seeking a Speech Therapist (PRN) to evaluate and treat residents in accordance with MD orders. The role involves comprehensive assessments, plan development, and close documentation to support patient care.

The ideal candidate holds an MA/MS in communication disorders recognized by ASHA, a current NC license, and has completed CFY. This PRN position offers flexible hours within a collaborative rehabilitation team.

Qualifications

• Graduate of an accredited university with an MA or MS in communication disorders recognized by ASHA and state board.
• Holds current license and/or registration in the state (as applicable).
• Successful completion of CFY.

Responsibilities

• Provide comprehensive speech therapy evaluations as per MD orders.
• Document findings on the standardized evaluation format neatly, accurately and adhering to guidelines.
• Develop a comprehensive treatment plan with long and short term goals, frequency, duration, and modalities.
• Ensure MD orders are obtained for evaluations, treatments, recertifications and discharges.
• Deliver comprehensive treatment to patients using approved modalities and adjust as needed.
• Document patient progress in approved format accurately and timely.
• Maintain documentation for treatment plans, discharges, and in-services as indicated.
• Attend facility and rehab meetings and participate in interdisciplinary team activities.
• Supervise junior staff or students per state regulations and co-sign notes where required.

Education

MA/MS in communication disorders (ASHA-recognized)
