Job summary

Evaluates patients, develops treatment plans and provides treatment for medically necessary speech therapy services addressing a patient's deficits in speech, language, cognition, voice and dysphagia in the home setting. The Speech Pathologists demonstrates the knowledge and skills necessary to provide care for the population served.

This role requires collaborating with a multidisciplinary team, covering weekends and holidays when needed, and documenting evaluations and progress while ensuring

Qualifications

• CCC-SLP certification with ASHA or active supervised fellowship year with eligibility for certification within one year of employment.
• Active license in the state of practice at time of hire.

Responsibilities

• Evaluates assigned patients through formal and informal diagnostic assessments to determine intervention needs in speech pathology.
• Demonstrates knowledge of age-specific factors from infant to geriatrics as per unit guidelines.
• Develops and implements individualized treatment programs for patients needing services.
• Establishes and updates intervention goals based on ongoing evaluations and promotes health through therapeutic activities.
• Provides direct therapy services in various settings and plans with patient capabilities in mind.
• Coordinates therapy with rehabilitation, medical, and nursing plans for each patient.
• Prepares evaluation results, treatment plans, progress notes, and related reports.
• Communicates with patient, family, physician, and other team members about status and needs.
• Builds rapport with interdisciplinary team and family throughout the intervention program.
• Participates in department program development and monitors clinical internships with students.
