Job summary

Enhance Rehabilitation is seeking a qualified Speech Language Pathologist to join our team in a SNF environment. You will evaluate and treat patients by following an individualized plan of care and collaborate with the treatment team to monitor progress.

The ideal candidate has experience with standardized assessments and swallowing evaluations, and can develop and adjust therapy plans to improve patient outcomes within a dynamic setting.

Qualifications

• Experience providing patient care in a SNF setting.
• Experience with communicating with patients, families and the care team.
• Experience administering speech/language assessments and monitoring progress.

In this role, you’ll evaluate and treat patients by following an individualized plan of care following an evaluation.

• Demonstrate the knowledge and skills necessary to provide patient care appropriate in a SNF setting.
• Strong written, oral and interpersonal skills work well with others and to communicate effectively with patients, families, and other treatment team members when gathering and exchanging patient condition information.
• Experience with administering standardized speech/language assessments, conduct swallowing evaluations, developing treatment plans and monitoring patients' progress.
