Job summary

St. Francis Healthcare Center in Hayward, CA is seeking a compassionate Speech-Language Pathologist to join our rehabilitation team on a PRN basis. You will evaluate and treat residents with speech, language, cognitive, and swallowing disorders to help them regain independence.

The position offers a supportive interdisciplinary environment, flexible scheduling, and opportunities to advance in a modern Skilled Nursing facility while delivering patient-centered care.

Qualifications

• Current Speech-Language Pathologist license in California.
• CCC-SLP certificate preferred; strong CF candidates may be considered.
• Experience in a skilled nursing facility or post-acute rehabilitation setting preferred.
• Knowledge of dysphagia assessment and treatment.
• Excellent communication, organization, and interpersonal skills.
• Ability to work independently while contributing to a collaborative team environment.

Responsibilities

• Evaluate residents with speech, language, cognitive, voice, and swallowing disorders.
• Develop and implement individualized treatment plans based on patient needs and physician orders.
• Provide skilled therapy services to improve communication, cognition, and swallowing function.
• Monitor patient progress and adjust treatment plans as appropriate.
• Educate residents, families, and caregivers on treatment goals and home strategies.
• Maintain accurate, timely, and compliant documentation.
• Collaborate with physicians, nursing staff, therapists, and other healthcare professionals to coordinate patient care.
• Ensure compliance with all federal, state, and facility regulations.
