Job summary

Shadelands Post Acute in Walnut Creek, CA, is seeking a compassionate Speech Language Pathologist (SLP) for PRN scheduling to join our rehabilitation team. You will deliver speech, language, cognitive, and swallowing services, develop goals, and coordinate with physicians and the interdisciplinary team to optimize patient outcomes.

Qualifications

• Current and active Speech Language Pathologist (SLP) license.
• Demonstrated clinical, communication, and interpersonal abilities.
• Ability to work effectively on an interdisciplinary healthcare team.
• Experience with patient assessment, treatment planning, and clinical documentation.
• Compassionate, professional, patient-centered demeanor.
• Strong organizational skills and attention to detail.

Responsibilities

• Develop individualized treatment plans and obtain physician authorization.
• Provide SLP services per physician orders and plans.
• Evaluate patients and set goals based on clinical findings.
• Monitor progress and communicate changes to the team.
• Participate in care conferences and care coordination activities.
• Complete evaluations, notes, and discharge documentation accurately.
• Educate patients and caregivers on strategies and safety.
• Assist with discharge planning and transitions of care.
• Support staff education and clinical development.
