Shadelands Post Acute is seeking a compassionate Speech Language Pathologist (SLP) - PRN to join our rehabilitation team in California. The SLP will provide comprehensive speech, language, cognitive, and swallowing services, develop treatment plans, and collaborate with an interdisciplinary team to optimize patient outcomes.

Qualifications

• Current and active Speech Language Pathologist (SLP) license.
• Strong clinical, communication, and interpersonal skills.
• Ability to work effectively as part of an interdisciplinary healthcare team.
• Experience with patient assessment, treatment planning, and clinical documentation.
• Compassionate, professional, and patient-centered approach.
• Strong organizational skills and attention to detail.

Responsibilities

• Develop individualized treatment plans and obtain physician authorization.
• Provide speech-language pathology services per orders and plans.
• Evaluate patients and set goals based on findings.
• Monitor progress and communicate with supervisors and team.
• Participate in care conferences and rehabilitation meetings as needed.
• Complete evaluations, daily notes, progress notes, and discharge docs.
• Document treatment changes per policies and payer requirements.
• Educate patients, families, and staff on therapeutic strategies and safe swallowing.
