Responsibilities

• Screen/evaluate patients with communication, cognitive, or swallowing disorders and develop care plans.
• Provide rehabilitative and medically necessary treatment per standards.
• Evaluate treatment outcomes and adjust interventions accordingly.
• Counsel patients and families, and coordinate with other providers.
• Prepare comprehensive discharge summaries as required.
• Document all clinical information in the electronic health record.
