  • Collaborate with nurses, occupational therapists, physical therapists, dietitians, and social workers to create a comprehensive care plan.
  • Maintain thorough records of assessments, treatment plans, progress notes, and discharge summaries.
  • Educate residents and families about therapy goals, strategies to maximize quality of life, and swallowing considerations.
  • Provide palliative care support focusing on communication and swallowing needs.
