Job summary

Lock Haven Rehabilitation & Senior Center seeks a dedicated PRN Speech-Language Pathologist (SLP/CCC) to join our in-house rehabilitation team in a SNF/LTC setting. You will evaluate residents with communication, cognition, and swallowing disorders and develop individualized treatment plans.

Work with an interdisciplinary team, provide skilled therapy, educate residents and families, and document progress timely.

Qualifications

• Master's degree in Speech-Language Pathology required.
• Current Pennsylvania SLP license.
• CCC-SLP certification required.

Responsibilities

• Evaluate residents with speech, language, cognitive, voice, and swallowing disorders
• Develop and implement individualized treatment plans
• Provide skilled therapy services to improve communication, cognition, and swallowing function
• Educate residents, families, and caregivers regarding treatment goals and progress
• Complete timely and accurate documentation
• Collaborate with physicians, nursing staff, and rehabilitation team members
• Participate in interdisciplinary care planning and discharge recommendations
