Responsibilities

• Screen and evaluate patients with physical functioning disorders and wound care; develop care plans.
• Provide rehabilitative, skilled, and medically necessary treatment interventions.
• Assist with assessments and discharge summaries per regulatory standards.
• Administer rehabilitation procedures including exercises, manual techniques, modalities, and wound care.
• Offer counseling to patients, families, and other providers regarding physical disorders.
• Document comprehensive discharge summaries ensuring regulatory compliance.
