Job summary

Avem Health Partners in Fairfax, OK seeks a Speech Language Pathologist to evaluate and treat residents and patients with speech, language, swallowing, and cognitive disorders in accordance with state and federal guidelines.

You will develop and implement individualized treatment plans, document progress, and collaborate with the healthcare team while maintaining strict safety and infection control practices. A valid Oklahoma SLP license and BLS are required; master’s degree is preferred.

Qualifications

• Graduate from an accredited school of Speech Pathology; Master’s degree preferred.
• Three (3) years clinical experience preferred.
• Current Oklahoma license for Speech Language Pathology (SLP).
• Current BLS (or obtained within 30 days of hire).

Responsibilities

• Administers patient care and procedures in this department.
• Evaluates and diagnoses speech/language deficits.
• Develops and implements individualized treatment plans.
• Provides direct therapy and revises plans as needed.
• Documents progress and outcomes per department policy.
• Communicates with patients and healthcare team.
• Assesses communication problems with standardized tests.
• Ensures safety and infection control practices.
