Job summary

Enhance Rehabilitation is seeking a qualified Speech Language Pathologist to join our dynamic team. You will evaluate and treat patients by following an individualized plan of care after an initial evaluation in a SNF setting.

Demonstrate the knowledge and skills to provide patient care in a SNF environment, with strong written and verbal communication, and collaboration with families and the treatment team. You will conduct standardized assessments, swallowing evaluations, and monitor progress.

Qualifications

• Licensed speech-language pathologist with SNF experience preferred.
• Proficient in administering standardized speech and language assessments.
• Ability to develop and monitor individualized treatment plans.

In this role, you’ll evaluate and treat patients by following an individualized plan of care following an evaluation.

• Demonstrate the knowledge and skills necessary to provide patient care appropriate in a SNF setting.
• Strong written, oral and interpersonal skills work well with others and to communicate effectively with patients, families, and other treatment team members when gathering and exchanging patient condition information.
• Experience with administering standardized speech/language assessments, conduct swallowing evaluations, developing treatment plans and monitoring patients' progress.
