Enhance Rehabilitation is seeking a qualified Speech Language Pathologist to join our dynamic team in Illinois. As an Enhance Rehabilitation Speech Therapist, you will evaluate and treat patients by following an individualized plan of care developed from a comprehensive evaluation, with emphasis on evidence-based practice.

You will provide patient care in a SNF setting, communicate with patients, families and the treatment team, and administer standardized speech and language assessments,

Qualificações

• Experience providing patient care in SNF settings.
• Ability to gather and exchange patient condition information with families and care teams.
• Experience with standardized speech/language assessments and swallowing evaluations.

Responsabilidades

• Evaluate and treat patients by following an individualized plan of care.
• Develop and monitor treatment progress and adjust plans as needed.

In this role, you’ll evaluate and treat patients by following an individualized plan of care following an evaluation.

• Demonstrate the knowledge and skills necessary to provide patient care appropriate in a SNF setting.
• Strong written, oral and interpersonal skills work well with others and to communicate effectively with patients, families, and other treatment team members when gathering and exchanging patient condition information.
• Experience with administering standardized speech/language assessments, conduct swallowing evaluations, developing treatment plans and monitoring patients' progress.
