Job summary

Stern Therapy Consultants is hiring a PRN Speech-Language Pathologist at Bear Mountain in Sudbury, MA. You will evaluate and treat adult communication, cognitive-communication, and swallowing disorders in a skilled nursing setting, tailoring goals to residents’ daily routines and rehabilitation needs.

You’ll provide therapy, coordinate with nursing and dietary staff, and document progress for care conferences and discharge planning.

Qualifications

• Active Massachusetts Speech-Language Pathologist license.
• Strong assessment and treatment skills for adult communication, cognitive-communication, and swallowing disorders.
• Effective interdisciplinary communication and timely documentation.
• Experience in skilled nursing or geriatric rehabilitation preferred.

Responsibilities

• Complete evaluations of speech, language, cognitive‑communication, and swallowing to establish clinical priorities.
• Develop individualized treatment goals grounded in residents’ daily routines and rehabilitation needs.
• Provide therapy for communication disorders and functional challenges involving attention, memory, and problem‑solving.
• Assess swallowing function and implement appropriate interventions, coordinating care recommendations with nursing and dietary staff.
• Coach residents, families, and caregivers on strategies that extend the value of therapy beyond each session.
• Maintain clear clinical documentation, measure progress, and contribute to care conferences and discharge planning.
