Job summary

The Avenues Transitional Care Center is seeking a licensed Speech Language Pathologist on a PRN basis in San Francisco, CA. You will conduct comprehensive evaluations, develop treatment plans, and implement evidence-based interventions for speech, language, cognition, and swallowing disorders in a skilled nursing setting.

This role offers true PRN scheduling with a premium per-diem rate of $60.00 to $75.00 per hour, flexibility to work around your availability, and a collaborative rehabilitation

Qualifications

• Current, active California SLP license.
• Master's or Doctoral degree in Speech-Language Pathology from an accredited program.
• Current CPR/BLS certification and compliance with CE requirements.

Responsibilities

• Conduct comprehensive speech-language and swallowing evaluations per physician orders.
• Assess and diagnose speech, language, voice, fluency, cognitive-communication, and dysphagia disorders.
• Develop and implement individualized plans of care with measurable goals.
• Provide skilled interventions addressing communication, cognition, and swallowing safety.
• Lead discharge planning and support safe transitions of care.
