Summary

We are hiring for a Weekend PRN Registered Nurse Case Manager in Hospice at Elite Hospice in Mena, AR, part of LHC Group. Responsibilities

As the RN Case Manager, you will be responsible for delivering the Plan of Care for each hospice patient, evaluating performance, and making necessary improvements. Your duties include:

• Conducting initial nursing evaluations within 48 hours of referral to determine eligibility.
• Assessing the patient's and family's physical, psychosocial, and environmental needs, with re-assessments at least every 15 days.
• Initiating and coordinating the plan of care, maintaining accurate clinical notes.
• Documenting problems, goals, interventions, and responses to care.
• Collaborating with the patient/family, attending physician, and IDG members in daily care provision.

Education and Experience

Requirements include:

• At least one year of clinical experience.
• Current RN licensure in the practice state.
• Current CPR certification.
• Valid driver's license, insurance, access to a dependable vehicle or public transportation.
