Job Responsibilities

1. Provide direct patient care using the nursing process, adhering to scope and standards of practice.
2. Perform care interventions including medication administration, specimen collection, and clinical treatments.
3. Document patient care accurately.
4. Monitor, record, and communicate patient conditions to the care team, physicians, patients, and families.
5. Assist patients with activities of daily living such as hygiene, elimination, nutrition, and mobility.
6. Coordinate patient care across disciplines, including transfers, discharges, referrals, and psychosocial support.
7. Assess and educate patients and families based on their learning needs, age, culture, and circumstances.
8. Advocate for the physical and emotional well-being of patients.
9. Perform additional duties as assigned, potentially including duties of CNA, MA, or LPN.
10. Participate in staff meetings, training, projects, and committees.
11. Follow policies and procedures of Highpoint Health with Ascension Saint Thomas.
12. Work scheduled shifts, including overtime and call schedules as directed.
13. Maintain a professional appearance per dress code.
14. Report occupational exposure to bloodborne pathogens.
15. Report to the Director of Skilled Care.

Qualifications

Minimum Education

Associate's degree required.

Certifications

License

Licensed Registered Nurse in Tennessee or from a Compact State with proper Tennessee paperwork filed within 30 days of hire.
