Qualifications

• Must possess a current RN license in the state of practice.

Responsibilities

• Assess patient needs across physical, psychological, social, spiritual, educational and discharge planning dimensions.
• Formulate and revise a plan of care in collaboration with the multidisciplinary team.
• Implement and coordinate care, delegating tasks and supervising other staff.
• Evaluate patient and family responses to care and adjust plans accordingly.
• Document assessments, plans and patient responses in the medical record.
