Adaptive Home Health is hiring a Registered Nurse (RN) - PRN to deliver superior in-home care in Detroit. The role emphasizes traveling to patients’ homes, performing comprehensive assessments, and documenting care with our AI-native platform.

Qualifications

• Active Michigan RN license required.
• Home health experience strongly preferred.
• Valid driver's license and reliable transportation.

Responsibilities

• Comprehensive in-home assessments and skilled documentation (OASIS, SOC/recert/ROC, discharge).
• Direct, high-acuity patient care, with a focus on wound care.
• Building and managing the plan of care with physicians; supervising LPNs and HHAs on your cases.
• Being the face of Adaptive to patients, families, and the referral sources who feed your pod.
