Our Hospice of Chattanooga team is seeking a compassionate and detail-oriented PRN Registered Nurse Case Manager (RNCM) to make a meaningful difference in the lives of patients and families in and around the Jasper Area.

The RN Case Manager will plan, organize, and direct hospice care using the nursing process—assessment, planning, interventions, implementation, and evaluation—and interact effectively with patients, caregivers, families, and interdisciplinary team members while maintaining professional standards.

Responsibilities
  1. Respond to clinical referral information with care and expertise, providing compassionate service.
  2. Partner with the Clinical Director to assess client eligibility for hospice services.
  3. Coordinate and deliver comfort-focused care, ensuring quality of life.
  4. Apply nursing skills to provide top-notch, evidence-based care.
  5. Educate and advocate for patients and families, fostering active participation in care planning.
  6. Participate in call duties, demonstrating our commitment to availability and support.
Minimum Qualifications
  • Current unencumbered RN license.
  • At least 1 year of RN experience in a medical-surgical or acute care setting; hospice experience is a plus.
  • Proficiency in computer use, including EMR systems.
  • Current CPR certification, if required by state.
Compensation

The pay range is $31.00 - $36.00 per hour, based on experience and qualifications. Final compensation will be discussed during the interview process.
