What You’ll Do:

Conduct assessments and create personalized care plans

Monitor and adjust treatment based on client needs

Educate patients and families on care instructions and follow-ups

Perform supervisory visits every 90 days (or every 60 days for CAP cases)

Collaborate with home health aides and field staff to ensure quality care

Travel to client homes in High Point, Archdale, Thomasville, and surrounding areas

Keep accurate records and report changes in client condition

Requirements:

Active RN license in NC or compact/multi-state license

Graduate of an accredited nursing program

Minimum of 2 years of nursing experience (home health/case management a plus!)

Comfortable using EMR systems and mobile documentation

Reliable transportation and flexibility to travel for visits

Strong communication and clinical judgment skills
