Description

This position exists to provide comprehensive, value-based, medication-related care to achieve definite outcomes that improve a patient's quality of life. The incumbent collaborates with healthcare professionals in shared responsibility for patient drug therapy outcomes. Key duties may include ensuring safety and effectiveness of medication use, processing and dispensing medications per order or prescription, medication therapy monitoring, regulatory compliance, overseeing pharmacist extenders, maintaining medication security, and protecting patient confidentiality. Additionally, the pharmacist provides drug information support to medical staff, ensures continuity of care, and identifies potential challenges in common situations.

Minimum Requirements:

  • Current Indiana license or eligibility for Indiana licensure as a pharmacist.
  • Successful completion of an ASHP-accredited PGY1 and/or PGY2 pharmacy residency may be required.
