Job summary

Continuum Hospice is seeking a Per Diem Compassionate Resource Nurse (RN) to provide patient care in and around the Concord area. This role supports the hospice team with pre-assessments, admissions, and education in patients' homes or healthcare facilities.

The Resource Nurse collaborates with the intake team, referring physicians, and the Hospice Medical Director to assure appropriate referrals and required certifications. Schedule includes Thursday to Sunday PRN rotations.

Qualifications

• Hold current unencumbered license as a Registered Nurse.
• Bachelor's degree in nursing OR one year of professional nursing experience within the last three years.
• Hospice/home health experience preferred but not required.
• Must be computer proficient with typing and EMR background.
• Possess and maintain CPR certification.
• Current driver's license, vehicle insurance, and access to dependable transportation.

Responsibilities

• Reviews clinical referral information and responds to requests; collaborates with Clinical Director on eligibility for hospice.
• Provides information, pre-assessment, and admission services to referred patients in the home or facility.
• Coordinates with Intake, referring physician, and Hospice Medical Director to ensure referrals are appropriate and certifications obtained.
• Applies nursing principles to treatment, provides care, emotional support, and education to patients and families; supports plan of care goals.
• Participates in on-call duties as assigned by Clinical Director and Program Director.

Continuum Hospice is currently hiring a Per Diem Compassionate Resource Nurse (RN) to provide patient care in and around the Concord Area!

Our Resource Nurses (RN) are responsible for assuring that the Hospice programs are effectively represented to patients, families, and others who have an interest in hospice through pre-assessments, information sharing and/or admissions in a Patient's home or in a health care facility. This position works with the intake team, referring physician, and other referral sources to promote the hospice philosophy of care.

Schedule: PRN Thursday, Friday, Saturday & Sunday Rotation

Territory: Contra Costa & Alameda County. Occassional Coverage in Napa and Solano County

Responsibilities

• Reviews clinical referral information and responds to request and inquires as appropriate. Collaborates with Clinical Director regarding the eligibility and appropriateness of client for hospice services.
• Provides information, pre-assessment, and admission services to patients who have been referred to the hospice; conducting pre-assessments visits in the home or healthcare facility as necessary.
• Communicates with the Intake Department, the referring physician and the Hospice Medical Director to assure appropriateness of each referral and that the necessary certifications are obtained.
• Applies nursing principles and techniques in the treatment of patients. Provides nursing care, emotional support and education to patients and families. Encourages patients and family members to participate in the patient's plan of care to meet established goals.
• Participates in On-Call duties as assigned and defined by Clinical Director and Program Director.

Qualifications

• Hold current unencumbered license as a Registered Nurse.
• Bachelor's degree in nursing from a program accredited by the National League for Nursing; OR
  • One (1) year experience as a professional nurse within the last three (3) years.
• Hospice/home health experience preferred but not required.
• Must be computer proficient in typing and various programs, including background in EMR.
• Possess and maintains current CPR certification.
• Current driver's license, vehicle insurance, and access to dependable transportation.

$65.00 - $67.00 per hour (Average Pay Range). The pay range listed represents a general guideline for the role and is not a guarantee of the final offer. Compensation will be determined based on the selected candidate’s relevant experience and the specific responsibilities of the position. Final compensation rate will be discussed and confirmed at the conclusion of the interview process.
