Concerto Renal Services is an industry leader in nursing home dialysis, focused on reimagining dialysis care through a community-centered model. We are seeking a PRN Social Worker to join our team, providing psychosocial evaluation and counseling to patients in Skilled Nursing Facilities or rehabilitation centers, collaborating with multidisciplinary teams, and documenting care in accordance with facility policies and Medicare requirements.

Essential Functions
  1. Conduct psycho-social assessments and develop treatment plans aligned with CMS guidelines.
  2. Complete KDQOL and other required tools.
  3. Document monthly in patients' charts.
  4. Educate patients on their rights, responsibilities, and grievance procedures.
  5. Track patients for QAPI and participate in related activities.
  6. Maintain professional relationships within the team and participate in staff meetings and training.
  7. Develop educational materials for patients and staff.
  8. Engage in special projects as needed.
Qualifications
  1. Master's degree in social work.
  2. Licensed as an LSW or LCSW.
  3. Unencumbered driver's license.
  4. Healthcare or dialysis-related experience preferred.
  5. Proficiency with MS Word, Excel, Outlook, and learning specific applications within 60 days.
Physical Demands
  1. Ability to lift at least 20 lbs.
  2. Attention to mental, visual, and auditory details.
  3. Work environment typical of healthcare settings.
  4. Travel between facilities required.
