Job summary

Concerto Renal Services is seeking a PRN Social Worker to provide psychosocial evaluations, counseling, and quality-of-life support for dialysis patients in Skilled Nursing or rehab facilities. You will assess needs, develop care plans with the interdisciplinary team, and document outcomes to meet CMS guidelines.

Responsibilities include counseling on dialysis and transplant options, leading care plan meetings, and coordinating with facility teams while traveling between facilities.

Qualifications

• Master’s degree in social work (MSW) required.
• Must hold a valid state license; clinical not required.
• Unencumbered driver’s license required.
• Healthcare or dialysis-related background preferred.
• Intermediate computer skills with MS Word, Excel, Outlook and knowledge of Concerto-specific applications within 60 days of employment.

Responsibilities

• Perform psychosocial assessments (initial, 90-day, annual, and as-needed) and develop individualized care plans with the interdisciplinary team.
• Provide counseling and education on dialysis, transplant options, and advance directives; support treatment adherence and quality of life.
• Lead monthly care plan meetings with patients and clinical staff; coordinate with facility teams, EQRS specialists, and regional managers.
• Maintain EMR documentation of assessments, interventions, and follow-ups; track productivity and outcomes; ensure CMS compliance.
• Advocate for patients and families; assist with outpatient dialysis placement and discharge planning; connect with community resources.

This role provides psychosocial evaluations, counseling, and quality-of-life support to dialysis patients in Skilled Nursing or rehab facilities. As part of the interdisciplinary team, the PRN Social Worker addresses psychosocial needs, ensures timely care planning per CMS guidelines, and maintains detailed documentation to meet QAPI standards.
