Job summary

Care Connect at Home in Boca Raton, FL is seeking an experienced Registered Nurse (RN) to join our team on a PRN / as-needed basis. This role offers flexible supplemental work with no guaranteed hours, scheduled and compensated as nursing needs arise within the agency.

Ideal candidates are licensed RNs with Florida credentials, capable of independent work, and comfortable traveling to client locations throughout Palm Beach County.

Qualifications

• Active, unrestricted Florida RN license.
• Home care, home health, or senior care experience preferred.
• Strong assessment and documentation skills.
• Reliable transportation and ability to travel to clients when needed.
• Professional, dependable, responsive, and comfortable working independently.

Responsibilities

• Conduct initial and follow-up client assessments as needed.
• Develop and update client care plans.
• Perform supervisory visits and reassessments when required.
• Review client conditions and identify changes in care needs.
• Provide clinical guidance related to client care.
• Maintain accurate and timely documentation.
• Assist the agency with RN-related clinical and compliance requirements.
• Communicate professionally with clients, families, caregivers, and agency leadership.

Care Connect at Home is a family-owned and operated home care agency seeking an experienced and dependable Registered Nurse (RN) to join our team on a PRN / as-needed basis.

This is an ideal opportunity for an RN looking for flexible supplemental work without a set weekly schedule. There are no guaranteed hours. The RN will be scheduled and compensated as nursing needs arise within the agency.
