Job summary

Village Caregiving in Butte, MT is seeking a PRN Registered Nurse to join our home care team. You will visit clients in their homes to document medical history, conduct health assessments, and develop personalized care plans for caregivers.

The schedule is flexible and the RN may occasionally train new caregivers. Pay is $60 per hour for up to 2 hours per client assessment. This role requires an active RN license, CPR certification, and reliable transportation.

Qualifications

• Graduate of an accredited Nursing program (Diploma/ASN/BSN).
• Current RN license in the state of Montana.
• CPR certification in good standing.
• Compassionate, friendly demeanor with excellent communication skills.
• Valid driver’s license and reliable transportation.
• Ability to work a flexible schedule and travel locally.

Responsibilities

• Visit patients in their homes for one-on-one visits.
• Complete initial and ongoing assessments to identify needs.
• Develop a care plan for caregivers to implement safely.
• Educate patients and families on home health care strategies.
• Provide effective communication with patients, families, and professionals.
• Fill out VA/Medicaid Nurse Assessment documents and upload to CRM.
• Maintain timely communication with the Executive Director about schedules.
• Notify the Executive Director at the end of each pay period of assessments completed.

The RN will visit clients' homes in and around Butte, MT to document medical history, conduct health assessments, and develop personalized care plans for caregivers. The nurse is also requested on occasion to train new caregivers. The schedule for the position is flexible. Pay for this position is $60/hour for up to 2 hours per client assessment.

Responsibilities

• Visit patients in their homes for one-on-one visits
• Complete initial and ongoing assessments to identify the physical, psychosocial, and environmental needs of home health patients
• Assess patients and their environment to develop a care plan in order for the caregiver to have a blueprint to know how to safely and competently take care of our patients
• Educate patients and their families on proper home health care strategies and procedures, and make recommendations for devices or tools that might improve the patient’s quality of life
• Provide effective communication to patients/clients, their family members, team members, and other health care professionals
• Fill out VA and Medicaid Nurse Assessment documents on a supplied tablet the same day as assessment. Upload Assessment to CRM.
• Keep timely communication with the Executive Director on schedule for upcoming and current assessments.
• Notify the Executive Director at the end of each pay period list of assessments completed.

Qualifications

• Graduate of an accredited Diploma, Associate, or Bachelor’s School of Nursing, or vocational nursing
• Current state license as an RN
• Proof of current CPR certification
