Job summary

Responsibilities

• Modify patient plans of care based on data collection and individual needs.
• Use evidence-based practice to provide patient care.
• Delegate per scope of practice and verify the plan with interprofessional team and patient/caregiver(s).
• Provide education based on learning needs and involve family.
• Promote effective communication among the interprofessional team and patients.
