Position Objective:

Job Summary:

As a PRN RN of Kindful Health, you will work with the Clinical Director to assist a care team including RNCM(s), LPN(s), Aides, MSW, and Chaplain to provide best in class care to a reasonably sized caseload. The RNCM will attend IDT meetings every 2 weeks with updates from the care team and coordinate with the Clinical Director and Medical Director to ensure the highest quality care.

Essential Education and Experience:

Must be a graduate of an approved school of professional nursing. Must have at least 2 years of nursing experience in the Hospice service line in the state of Texas.

Responsibilities & Duties:

• Adhere to all regulatory requirements and professional standards.
• Follow policies and procedures consistent with recognized standards of professional practice.
• Identify system problems or inefficiencies that may negatively affect patient care.
• Manage clinical excellence within your patient census.
• Provide high levels of communication both internally with the Kindful team and externally with patients and families to ensure top CAHPS scores.
• Collaborate with your Clinical Director, Medical Director, and the entire team to provide a "concierge" level hospice experience.

Qualifications & Skills:

The ideal candidate demonstrates mastery of all facets of hospice care and has a communication style that motivates team members to excel. They are equally comfortable in the field providing patient care and in mentoring colleagues.

Compensation:

$45 per hour
