Job summary

SYNERGY HomeCare is seeking a PRN Registered Nurse (RN) to provide in-home nursing care in Chantilly, VA. You will assess needs, develop and implement care plans, administer medications, and educate families while maintaining documentation.

Candidates should have 1-2 years of RN experience, an active RN license, strong communication, and the ability to work independently within a multidisciplinary team. Flexible schedule and reliable transportation are required; background checks and references

Qualifications

Responsibilities

• Provide skilled nursing care to clients in their homes.
• Assess client needs, develop and implement care plans, and update as necessary.
• Administer medications, perform wound care, and other nursing duties as required.
• Educate clients and their families on health care needs and preventive measures.
• Maintain accurate and timely documentation of client care.
• Adhere to all health and safety guidelines and company policies.
