Job summary

Nexus Family Healing in Portland, Oregon is seeking several PRN Registered Nurses for our Walden Crossing residential program. This on-site role serves youth ages 10-17 with mental and developmental disabilities involved in the justice system in a secure setting.

You will collaborate with a multidisciplinary team to deliver comprehensive medical care, coordinate medications, and participate in required trainings.

Qualifications

  • Graduate of an accredited school of nursing.
  • Valid RN license.
  • QMHP qualified in Oregon.
  • 1+ year related nursing experience.
  • First Aid/CPR/Infection Control certification.
  • Valid driver’s license.
  • Must be able to meet the cultural needs of diverse individuals and communities.

Responsibilities

  • Collaborates with interdisciplinary teams regarding medical aspects of client admissions.
  • Communicates medical concerns and coordinates follow-up appointments/referrals with teams.
  • Collects and reviews clients’ medical data; ensures medical records are current and accurate.
  • Assesses nursing implications of data and communicates with physicians as needed.
  • Monitors acute and chronic medical conditions; provides treatment or consultation as needed.
  • In emergencies, gathers information, assesses status, and directs appropriate care or referrals.
  • Serves as a resource to staff on medical concerns; collaborates on individual treatment plans.
  • Verifies, counts, and administers medications; educates clients on side effects and interactions.
  • Maintains medication logs and ensures proper continuation during residency.

Education

Graduate of an accredited school of nursing
Valid Registered Nurse License
QMHP qualified in Oregon
1+ year related nursing experience
First Aid/CPR/Infection Control certification
Valid driver’s license
Nonviolent Crisis Intervention training

Schedule/Location/Pay
  • PRN On-site opportunity
  • Walden Crossing Address: 722 NE 162nd Avenue, Portland OR 97320
  • Non-hospital setting; small residential program
  • Working with youth clients - ages 10-17
  • Starting pay range: $47-$50 an hour!

Shift Differential
  • $1/hour differential for overnights during the week, Sunday night through Friday morning: (12AM - 7:59 AM)
  • $2/hour for day and evening shifts on weekends, Saturday morning through Sunday evening: (8AM - 11:59 PM)
  • $3/hour for weekend overnights, Friday night and Saturday night: (12AM - 7:59 AM)
Position Summary

Our Nurse (RN) promotes the health and well-being to Nexus Family Healing residents by providing client-centered health care services, in accordance with Nexus Family Healing policies, procedures, and protocols.

Primary Responsibilities
Resident Medical Care
  • Collaborates with interdisciplinary teams regarding medical aspects of client admissions.
  • Communicates medical concerns and coordinates necessary follow-up appointments, and/or referrals with interdisciplinary teams both internally and externally.
  • Collects and reviews clients' medical data and ensures that medical records are current and accurate.
  • Assesses nursing implications of all data pertaining to the resident and determines the need to communicate with the physician.
  • Monitors acute and chronic medical conditions of clients; provides treatment or consultation as needed.
  • In emergency situations, gather relevant information and assess client status. Provide appropriate care and/or treatment, direct others as necessary, and collaborate referral to a higher-level of care if appropriate.
Treatment Team Collaboration
  • Serves as a resource to staff and other interdisciplinary teams regarding client medical concerns.
  • Collaborates with the treatment staff to develop individual client treatment plans.
Implement, monitor, and coordinate medication administration to clients
  • Verify, count, and administer medications to residents.
  • Educates clients on medication side effects, adverse effects, and drug interactions.
  • Monitors clients daily for side effects, adverse effects, and/or drug interactions.
  • Ensures client prescriptions are filled and the appropriate amount is maintained through each client’s stay within the residential milieu.
  • Monitors medication logbooks for completion and accuracy.
Staff Meetings & Required Training
  • Completes all required trainings that are department/role specific & facility-wide requirements.
  • Maintains and stay up to date with trainings and certifications (i.e., online trainings, internal and external in-person trainings, CPR certification, nursing license, first aid and safety, medication administration, and infection control etc.
  • Participates in internal training opportunities with community partners.
Client Confidentiality
  • Ensures client information remains confidential and compliant with Nexus policies involving client privacy, informed consent and confidentiality standards for clients and their families.
Requirements
Required Education and Licensure
  • Graduate of an accredited school of nursing
  • Valid Registered Nurse License
  • QMHP qualified in Oregon
  • 1 + year related nursing experience
  • First Aid/CPR/Infection Control certification
  • Valid driver's license required.
  • Must have the ability to build strong relationships with diverse individuals or communities and understand their unique needs and challenges.
  • Must be open to meeting the cultural needs of individuals or communities, considering factors such as language, traditions, and values.
  • Successfully participate and complete a three-part Nonviolent Crisis Intervention exam and training
Preferred
  • 1+ year related nursing experience in a residential setting
