Job summary

Nexus Family Healing in Eugene, Oregon seeks a Registered Nurse to join our Youth Psychiatric Residential Treatment facility opening this summer. The RN will deliver client-centered care to residents ages 12–17, within a small on-site, non-hospital setting and in partnership with a multidisciplinary team.

The role emphasizes medical care coordination, medication administration, and confidentiality, with comprehensive benefits and a supportive ICARE culture.

Qualifications

  • Bachelor's degree in Nursing required.
  • Graduate of an accredited school of nursing.
  • Valid Registered Nurse License in Oregon.
  • QMHP qualified in Oregon.
  • 1+ year of full-time nursing experience.
  • First Aid/CPR/Infection Control certification.
  • Valid driver's license.

Responsibilities

  • Collaborates with interdisciplinary teams regarding medical aspects of client admissions.
  • Coordinates follow-up appointments and referrals with internal/external teams.
  • Reviews and maintains current medical records.
  • Assesses nursing implications of resident data and communicates with physicians as needed.
  • Monitors acute and chronic medical conditions; provides treatment or consultation.
  • In emergencies, gather information, provide care, and coordinate higher level of care as appropriate.
  • Serves as a resource to staff and other teams on medical concerns.
  • Participates in developing individual client treatment plans.
  • Administers medications and educates residents on side effects and interactions.
  • Maintains medication logs and adherence to policies.

Nexus Family Healing is looking to add a Registered Nurse to join our newest Youth Psychiatric Residential Treatment facility set to open this summer in Eugene, Oregon. Nexus Family Healing is expanding to Eugene, Oregon, with the opening of a new 14-bed psychiatric residential treatment facility (PRTF) set to open this summer. This location is in partnership with the Oregon Health Authority Behavioral Health Division and will serve justice-involved youth ages 12–17, experiencing acute emotional concerns, and serious and persistent mental illness. Position Summary

Our Nurse (RN) promotes the health and well-being to Nexus Family Healing residents by providing client-centered health care services, in accordance with Nexus Family Healing policies, procedures, and protocols.

Primary Responsibilities
Resident Medical Care
  • Collaborates with interdisciplinary teams regarding medical aspects of client admissions.
  • Communicates medical concerns and coordinates necessary follow-up appointments, and/or referrals with interdisciplinary teams both internally and externally.
  • Collects and reviews clients’ medical data and ensures that medical records are current and accurate.
  • Assesses nursing implications of all data pertaining to the resident and determines the need to communicate with the physician.
  • Monitors acute and chronic medical conditions of clients; provides treatment or consultation as needed.
  • In emergency situations, gather relevant information and assess client status. Provide appropriate care and/or treatment, direct others as necessary, and collaborate referral to a higher-level of care if appropriate.
Treatment Team Collaboration
  • Serves as a resource to staff and other interdisciplinary teams regarding client medical concerns.
  • Collaborates with the treatment staff to develop individual client treatment plans.
Implement, monitor, and coordinate medication administration to clients
  • Verify, count, and administer medications to residents.
  • Educates clients on medication side effects, adverse effects, and drug interactions.
  • Monitors clients daily for side effects, adverse effects, and/or drug interactions.
  • Ensures client prescriptions are filled and the appropriate amount is maintained through each client’s stay within the residential milieu.
  • Monitors medication logbooks for completion and accuracy.
Staff Meetings & Required Training
  • Completes all required trainings that are department/role specific & facility-wide requirements.
  • Maintains and stay up to date with trainings and certifications (i.e., online trainings, internal and external in-person trainings, CPR certification, nursing license, first aid and safety, medication administration, and infection control etc.)
  • Participates in internal training opportunities with community partners.
Client Confidentiality
  • Ensures client information remains confidential and compliant with Nexus policies involving client privacy, informed consent and confidentiality standards for clients and their families.
Requirements
Required Education and Licensure
  • Bachelor's degree required
  • Graduate of an accredited school of nursing
  • Valid Registered Nurse License
  • QMHP qualified in Oregon
  • 1 + years of full time related nursing experience
  • First Aid/CPR/Infection Control certification
  • Valid driver’s license required. Must meet state regulating agency and Nexus Home Office driving requirements
  • Must have the ability to build strong relationships with diverse individuals or communities and understand their unique needs and challenges.
  • Must be open to meeting the cultural needs of individuals or communities, considering factors such as language, traditions, and values.
  • Successfully participate and complete a three-part Nonviolent Crisis Intervention exam and training
Preferred Qualifications
  • 1+ year related nursing experience in a residential setting
