Responsibilities and Duties:

• Implement the plan of care in consultation with the physical therapist and other care team members.
• Communicate findings or changes regarding patients back to the physical therapist and care team.
• Educate patients and families on basic home safety and medical equipment safety.
• Be solution-driven, self-motivated, and inspiring to patients to reach their goals.
• Work independently without direct oversight.
• Recognize when to seek support and communicate challenges.
• Be familiar and comfortable with technology.

Qualifications and Skills:

• Associate Degree in Physical Therapy
• Current Certification in BSL/CPR
• Home Health Experience
• Valid Texas Driver's License and Automobile Insurance
• Knowledge of WellSky EMR software is a plus
