Job summary

Northwest Healthcare is seeking a licensed Physical Therapist to evaluate, plan, and administer rehabilitation services. You will establish individualized goals, provide treatments, and reassess progress while documenting results for physicians and the care team.

Qualifications

• Doctorate in Physical Therapy (DPT) required.
• 0-2 years of experience in a physical therapy setting required.

Responsibilities

• Conducts thorough Physical Therapy evaluations to assess patient needs and develops individualized treatment plans.
• Administers safe and effective therapy treatments, ensuring all interventions are appropriately documented.
• Reassesses and adjusts treatment plans within appropriate timeframes to align with patient progress and changing needs.
• Provides clear, concise, and accurate documentation of evaluations, treatment plans, progress notes, and billing information.
• Educates patients and their families on treatment plans, therapeutic exercises, and post-treatment care to support long-term recovery.
• Operates therapy equipment safely and maintains a clean, organized, and safe environment for patients and staff.
• Collaborates with referring physicians, peers, and interdisciplinary team members to ensure continuity and quality of patient care.
• Demonstrates population-appropriate competencies for all relevant patient demographics, tailoring care to individual needs.
• Maintains compliance with facility policies, including safety, infection control, and risk management procedures.
• Adapts to changes professionally and effectively, following the chain of command to address and resolve issues impacting patient care.
• Performs other duties as assigned.
