Job summary

Cornerstone Rehab is seeking a PRN Physical Therapy Assistant to deliver care to adult and geriatric patients across multiple centers. The role involves developing and implementing plans of care, documenting progress, and coordinating with a diverse team of clinicians.

The position requires a valid State PTA license and credentials, with SNF and/or acute care experience preferred. Travel to nearby centers is expected as needed, supporting our mission to meet patient outcomes across our network.

Qualifications

• Valid State PTA License and Credentials.
• SNF and/or Acute care experience.

Responsibilities

• Provide physical therapy to adult and geriatric patients.
• Prepare documentation, progress notes, and discharge summaries.
• Establish a plan of care in accordance with the patients' needs and capabilities.
• Travel to nearby centers as needed.
