Job summary

Duly Health and Care is seeking a PRN outpatient Physical Therapist in Lockport, IL. You will provide thoughtful, one-on-one care with clinical autonomy and a strong internal referral network.

Scheduling includes a minimum commitment of two Saturdays per month and additional weekday availability, with long-term opportunities within a large clinical network. Ideal candidates are graduates of an accredited PT program, licensed in Illinois, and able to work at least 2 hours per week.

Qualifications

• Graduate of an accredited Physical Therapy program and current Illinois PT licensure or eligibility.
• License must be in good standing and active to practice in Illinois.
• Must be able to work minimum 2 hours per week.

Responsibilities

• Provide one-on-one outpatient physical therapy evaluations and treatments.
• Schedule and cover Saturdays per month, with additional weekday hours as needed.
• Collaborate with PTs, OTs, PTAs, physicians and APPs to coordinate care.
• Deliver care with clinical autonomy while maintaining documentation and compliance.
