The Social Worker (MSW) at MUSC Community Physicians provides psychosocial assessment, discharge planning, and case management in an acute care psychiatric setting. This PRN role supports a 6-hour shift schedule on Saturdays and Sundays, bridging inpatient care with outpatient follow-up to improve patient outcomes.

Requires an MSW with LMSW/LISW licensure in South Carolina; BLS and Safety Care certifications within 90 days. Strong communication and collaborative skills are essential for success.

Qualifications

• Master's degree in social work accredited by the Council on Social Work Education.
• One year post graduate professional social work experience; three years preferred.
• LMSW/LISW licensure by the SC Board of Social Work Examiners; LISW-CP preferred.

Responsibilities

• Conduct psychosocial assessment of patients and caregivers.
• Coordinate discharge planning with health care team.
• Provide direct clinical social work services in an acute psychiatric setting.
• Engage in discharge planning and case management for complex caseloads.
• Identify post-discharge risks and implement interventions to mitigate them.
• Bridge discharge to outpatient providers and address patient questions.
