Continuum Rehab Group provides in-house therapy with Skilled Nursing facilities in Texas, New Mexico, Indiana, Arizona, and Missouri!

UP TO $38 Hourly! Consistent Hours!

Experience/Qualifications:

Licensed Physical Therapy Assistant. This position is open to therapists at all experience levels.

Some duties for this position include:
  1. Utilizes evidence-based practice to support clinical interventions.
  2. Provides rehabilitative, skilled and medically necessary treatment intervention to patients with physical functioning disorders per regulatory and clinical practice requirements under the supervision and direction of a Physical Therapist.
  3. Provides skilled therapy services to patients who need rehabilitation services.
  4. Assesses and documents patient’s progress and communicates with supervising Physical Therapist to adjust treatments accordingly.
  5. Provides consultation and counseling to patients, as well as families, caregivers, and other service providers related to physical disorders.
  6. Provides input to supervisor for discharge summary of services provided per regulatory and clinical requirements.
