Job summary

Community Physical Therapy is hiring a PRN Physical Therapist Assistant (PT) with Trach/Vent experience to support our Skilled Nursing Facility partners. Qualifications

• Active Illinois PT license required.
• Trach/vent experience preferred.
• SNF experience preferred.
• Familiarity with Net Health is a plus - training provided.

Responsibilities

• Provide PT services in Skilled Nursing Facility settings.
• Collaborate with care teams to deliver patient-centered care.
• Support PRN staffing and flexible scheduling.
