About the role: Enhance Rehabilitation is seeking qualified Physical Therapists to join our dynamic team. As a Physical Therapist, you will provide high-quality care to patients in a Skilled Nursing Facility (SNF) setting, evaluating and treating patients based on individualized care plans.

• Provide excellent patient care and customer service to patients and their families
• Collaborate with facility staff to ensure optimal patient care
• Evaluate new patients, develop care plans, and set measurable, functional goals
• Treat patients, address concerns, and educate on injury, progress, and outcomes
• Supervise Physical Therapist Assistants
• Re-evaluate patient progress, revise care plans, and adjust goals accordingly
• Complete accurate and timely documentation including evaluations, re-evaluations, discharges, and daily progress notes
