Job summary

Lakeshore Manor Nursing & Rehab in Slidell, LA is seeking a Physical Therapist (PT) to evaluate patients, create personalized treatment plans, and guide therapeutic exercises. You will work with a interdisciplinary team to support progression and independence, while ensuring timely documentation and billing.

Qualifications

• Licensed in Louisiana or actively pursuing licensure in physical therapy.
• Graduate of a CAPTE-accredited physical therapy education program.
• Experience in post-acute care or similar healthcare setting preferred.

Responsibilities

• Conduct patient evaluations and develop individualized treatment plans.
• Provide therapeutic exercises, functional training, and pain management.
• Educate residents and families on safe use of equipment and exercises.
• Collaborate with interdisciplinary teams to promote progress and independence.
• Complete documentation and billing accurately and timely.

Compensation: $47–$60/HR (Depending on experience)

Schedule

• PRN/On-Call shifts are available

What You’ll Do

• Conduct patient evaluations and develop individualized treatment plans
• Provide therapeutic exercises, functional training, and pain management
• Educate residents and families on safe use of equipment and exercise techniques
• Collaborate with interdisciplinary care teams to promote progress and independence
• Accurately complete documentation and billing in a timely manner

What We’re Looking For

• Licensed in Louisiana or a graduate of an approved Physical Therapist program or actively working toward licensure
• Graduate of a CAPTE Accredited Physical Therapy Education Program
• Experience in post-acute care or similar healthcare environment preferred
• Strong communication and problem-solving skills
• Commitment to collaboration and patient-centered care
